Certified Home Health Aide (HHA) – Per Diem

Affirmed Home Care is seeking compassionate and experienced Certified Home Health Aides (HHAs) for multiple per diem opportunities in Huntington and throughout Suffolk County.

We currently have multiple cases available with 6–8 hour shifts, offering flexible opportunities for HHAs looking to work close to home.

Available Schedules

  • Tuesday–Thursday: 4:00 PM–10:00 PM
  • Monday–Thursday: 9:00 AM–3:00 PM
  • Friday: 8:30 AM–4:30 PM

Multiple cases are available. Additional assignments may also be available based on your availability and preferred location.

Qualifications

  • Active New York State HHA Certification required
  • Minimum of one year HHA experience
  • Authorized to work in the United States
  • Valid driver's license preferred
  • Reliable transportation strongly preferred

Required Health Documentation

  • Physical examination completed within the past 12 months
  • PPD or QuantiFERON test completed within the past 12 months, or chest X-ray completed within the past 5 years
  • MMR immunization within the past 10 years
